Hey guys, a little about myself. I am a 28-year-old 5’8 male, been in and out of the gym most of my life. I would be consistent for a bit and then get discouraged and end up not going to the gym for a while and then repeating the cycle. Back in February, I decided to get disciplined and be consistent. I started at 150 lbs and bulked for 6 and a half months. I went from 150 to 175 lbs and my bench increased 50 lbs and my squat increased 40 lbs. I averaged about 4 lb gain each month. I had to stop bulking and start a cut because it appears most of that weight went right into my abdomen and nowhere else and started to look pregnant. I also haven’t noticed any visible muscle gain during that time.

Throughout my life I have always suspected having lower than normal testosterone levels. Even when I was 16 I got my levels checked and they came back at 250. Periodically I would get them tested throughout my 20s and it never came back over 400, it always fluctuated in the 200s and 300s. It has been a few years since getting tested, so I went ahead and got labs done again this month and it came back at 368. The TRT clinic I went to for the labs said it was low but not low enough to qualify for TRT unless I do a consultation with the nurse or doc and rule out any medical issues that may be causing it. I’ve been in and out of doctors’ offices for this issue and they all give me the same run down - eat healthier, work out more, sleep more, manage stress. I live a pretty active lifestyle, eat pretty healthy, in the gym at least 3x a week, and have a relatively stress-free life. The only thing I can think of that may be a contributing factor is I have what I believe to be short sleep syndrome. I get about 4-5 hours of sleep a night and it doesn’t matter what I do it is impossible to sleep any more than that. However, as long as I don’t get less than 4 hours of sleep, I feel perfectly fine and well rested, so I am unsure if it is an issue or not or a contributing factor to low T. I know my father was on TRT in his 40s.

I don’t have any signs and symptoms of being super affected by the lower testosterone, however, it does affect my sex drive. Sometimes it seems normal and other times I have an issue getting an erection with my gf and can go weeks with no morning wood - it fluctuates. Tomorrow, I have a doctors appoint with my PCP and am going to bring him my lab work and go over everything. I know once you get on TRT it is probably a lifelong thing, but I feel like it is in my future anyways, so I am curious about pursuing it.

Do you guys think this is something worth pursuing, and if so, any tips on what to tell a doc for referrals or possibly getting treatment? I know most docs will just say oh you fall within 200-1100 which is normal, so you don’t need it, or that I’m young and they are hesitant to put me on hormone therapy for fertility reasons. Any advice would be appreciated.

Post your blood work if you can so we can help with our opinions. There are lots of potential variables.

You could have an estrogen or prolactin issue.
(you want estro between around 30 and prolactin less than 30)

Also your Free T is waaay more important than total T.
(you want this to be between 30-60 ideally if you are seeking symptom resolution)

Maybe you simply need to try low dose Cialas before screwing with your hormones.

Also get yourself a apple watch to track your sleep to see the quality of those 4-5 hours.

You’re definitely way low for your age.

My opinion on “bulking” and “cutting” is its mostly a wasted effort. Most folks just get pudgy and then lose the pudge … wash rinse repeat.

Agree about posting labs. Wipe out your personal info and post them. Also agree about free test.

Have you tried a “power building” type routine where as you’re training the heavy compound lifts also? That will bolster your T. I’m of the opinion that a thought out split (tailored to your abilities and needs) will do wonders for strength AND physique.

Do you track your food intake? Until you’ve dialed in your macros…it’s kind of hard to see what’s working for you and what’s not. Be brutally honest with yourself.

Sleep… I got no advice… my sleep is all over the place…
 
Maybe I can bring up Cialis at the appointment as well. Good idea on the apple watch, I was thinking of getting one at some point anyways.

I can’t attach the screenshot of my testosterone lab result, but all it says is: Testosterone 368 ng/dl reference range 264-916
 
I will post the testosterone lab I had done. Just says testosterone on it. Is that something you have to ask for? I presumed a TRT clinic would preform blood work for anything related to testosterone. The other blood work I can’t get right now as the website is having issues.

Personally, I cannot gain strength while on maintenance calories whatsoever, nor while on a cut. I have been tracking my TDEE and counting calories and ever since starting the cut a month ago, I have gained no strength, just maintaining the weight on the bar. I didn’t have that issue while eating 500+ caloric surplus, strength was climbing, and weight being gradually added to the bar. I just think 500 surplus and 4 lb gain a month is too quick, I could dial it down to a 200 surplus and do a lean bulk to minimize the fat gain. From my experience and what I have tracked so far, strength gain has been directly related to my caloric intake. I’ve also experimented with protein too and found no difference with a lower protein intake compared to a higher. The only difference in my strength progression has only been from calories. As far as training, I do squats and bench heavy twice a week for most routines I have been on. The rep ranges used have always been 3x5 for those.

the calories and strength gain are more than likely correlation and not causal…big difference between the two.

Totally agree about learning your macros.

Ima xfit guy so i like the compound lifts that are complexed for total body work as well as higher time under tension.

Need to make sure you have at least 3 cardio sessions every week as well.

I hear ya on calorie restrictions and lose of strength. Same with losing body mass. Mass moves mass and your leverages will change.
 
@Nitrochicken do you have health insurance?

If so, an annual (full) physical is not unreasonable at your age…complete with a comprehensive blood panel. Usually insurance will pay for one a year.

You also have the option to “shop around “ for trt/hrt clinics. There’s a gazillion of them these days to include telehealth/online clinics.

Don’t forget, this is like any other commodity/service… you’re the customer and have the right to look for services that you need or desire.
 
I do and I got what I believe to be comprehensive labs done two days ago. Waiting for the website to work again so I can put the results here. Was just a bunch of listed things I’ve never even heard of before lol.

I also shopped around a little bit. I called this one clinic and they said that I qualified for TRT and with self-pay it would be 68 bucks a week. That includes lab work, test cyp, hcg, and any estrogen blocker if need be. It’s once a week injections
 
Ok cool. Just wipe out any personal identifying info.

68 a week is kind of pricey I think

Causation versus correlation…rule out the easiest things first before the life long treatment protocol which is a pricey lifestyle too.

Need to be very Diligent working this process if you want to know the answer.

Just my two cents.
